I am currently a participating provider for: Aetna, Blue Cross/Blue Shield, Cigna, EBMS, Beech Street, Medicaid, Medicare, Meritain, MultiPlan, and TRICARE.  

You are responsible for understanding your insurance benefits. If proof of insurance is not provided on the first visit, you will be responsible to pay the full amount for services rendered at the time of service until proof of insurance is provided. Visits are billed to insurance companies as a courtesy. 

Individuals with health insurance: co-pays and coinsurance payments are due in full at the time of service, and are required to be paid as part of your contract with your insurance carrier.

Typical insurance scenarios:

1. Deductible, then coinsurance
2. Deductible, then copay
3. Copay only

Uninsured individuals: if you do NOT have coverage by an insurance carrier, you may receive a discount for payment provided at the time of services rendered. 

Non-covered services: wellness, maintenance, fitness, and performance training visits. This fee is due at the time of service.
